PII Scanning Request Form

If you would like to make a request in regards to personally identifiable information (PII) scanning, please complete the web form below. ITS-SOS will be happy to assist your campus/college/department area. There is no charge for this service.

Only someone explicitly designated as a contact per the signed Memorandum of Understanding (MOU) covering the campus/college/department is authorized to make requests using this form. Repeated unauthorized submission of this form (e.g., abusive activity) may result in referral to the Office of Student Conduct or the Office of Human Resources. Once the request has been received, SOS may contact you for further details prior to servicing the request.

  Identity Finder MSI (Windows) and PKG (Mac) files
This is typically the first step in the process after SOS receives a signed MOU covering your campus/college/department. Please specify appropriate "tag" in comments section below. Tags take the format of College/Department or Administrative Area/Work Unit (e.g., ITS/SOS).
  Console: Tag Maintenance
Use this option to request tag renaming or removal from the Identity Finder console. Also use this option to request deletion of match and/or log data for a particular tag.
  Console: Endpoint Maintenance
Use this option to request endpoint renaming or removal from the Identity Finder console. Also use this option to request deletion of match and/or log data for a particular endpoint.
  Console: User Creation/Maintenance
Use this option to request user account creation or removal from the Identity Finder console.
  Console: Role Assignment/Maintenance
Use this option to link or unlink a user account to a role on the Identity Finder console.
  Product Support/General Questions
Use this option for general questions or product support regarding the Identity Finder products.
  Other
Use this option for anything that doesn't fit into the categories above.
